How We Can Support You

Wise Care Services offers services and supports to people with disabilities.

We provide flexible NDIS supports designed around each participant’s needs and goals, including:

Daily Personal Activities High Intensity (0104)

Complex supports such as enteral feeding, complex bowel care, tracheostomy care, ventilator management, and other high intensity clinical supports delivered by trained staff.

Daily Personal Activities (0107)

Support with personal care tasks including showering, dressing, grooming, toileting, and mobility assistance.

Assist Travel & Transport (0108)

Support provided to enable participants to travel safely to appointments, community activities, or social outings.

Community Nursing (0114)

Clinical nursing services including health assessments, medication management, chronic disease management, and complex health support delivered by a Registered Nurse.

Supported Independent Living (0115)

Supported Independent Living (SIL) support provided within a shared living arrangement to assist participants with daily routines, independence, household management, and social participation.

Life Skills Development (0117)

Capacity building supports to assist participants in developing independence in budgeting, cooking, communication, personal organisation, and decision-making skills.

Household Tasks (0120)

Assistance with cleaning, laundry, meal preparation, and maintaining a safe home environment.

Community Participation (0125)

Support to access and engage in community events, recreational and sports activities, education and skill development programs to promote inclusion and wellbeing.

Group Centre Based Activities (0136)

Support structured activities in a group setting at a centre or facility, where participants can develop arts and creative expression, skill development, fitness and wellness, build social connections, and increase independence.
